Your Right to a Good Faith Estimate
Under federal law, individuals who do not have insurance or who are not using insurance to pay for care generally have the right to receive a “Good Faith Estimate” explaining how much their health care services are expected to cost.
If you are eligible for a Good Faith Estimate, Oak & Ember Wellness, LLC will provide an estimate of the expected charges for psychotherapy services upon request or when required by law.
The estimate is not a contract and does not require you to receive services from Oak & Ember Wellness, LLC. Actual charges may differ if your treatment needs or services change.
If Your Bill is Higher Than Expected
If you receive a bill that is at least $400 more than your Good Faith Estimate from a provider or facility, you may have the right to dispute the bill through the federal patient-provider dispute resolution process.
Make sure to save a copy or photograph of your Good Faith Estimate.
